Dear Patients and Friends,

 It is with mixed feelings that I, Sheila A. Burke, M.D., announce my retirement from active medical practice as of May 23, 2014.  I will be forever grateful for the honor of your allowing me to share your lives for the past 33 years in my medical practice.  I am hoping to further pursue some hobbies and interests during my retirement. 

 Alaska Family Medicine Associates is closing.  After May 23, 2014 no prescription refills and treatment recommendations can be made.   The business office will remain open until May 30, 2014.  Annette Hewitt, A.N.P. has accepted a position at Alaska Native Tribal Health Consortium. 

 I am entrusting your confidential health records to USHealthWorks DBA Primary Care Associates when I retire.  They are an experienced and well respected group with excellent physicians, A.N.P.’s, and P.A.’s, including Mary Ann Foland, M.D., Irina Grimberg, M.D., Eric Miknich, M.D. and Kathrine Hardy, A.N.P.  I am confident they share our values of patient centered care.

  This should be a seamless transition to use this medical group for your continuing medical needs.  Primary Care Associates accepts and bills most insurance, and has four convenient locations with after-hour and weekend availability.  After June 1, 2014 your records will be located at Primary Care Associates, 4100 Lake Otis Parkway Suite 322, Anchorage, AK  99508, (907) 562-1234.  Of course you are free to see any medical provider of your choice.  If you choose to see another provider, please sign the enclosed authorization form and return it to my office as soon as possible so that we may get you a copy of your records.

 I am hoping for continued health and happiness for you and your families.  I will forever treasure our relationships, your loyalty and friendship that we have nurtured over the years.
